What "Natural" Means on a Toothpaste Box

Nothing enforceable. In its guidance for cosmetics manufacturers, the FDA writes that it "has not defined the term 'natural' and has not established a regulatory definition for this term in cosmetic labeling." The same page tells manufacturers not to use the word inside an ingredient statement at all, because ingredients must be listed by their common names without extra description.

It also heads off the inference most shoppers make. Choosing ingredients from sources you consider organic or natural, the agency says, "is no guarantee that they are safe."

So the word does whatever the brand needs it to do: plant-derived actives, the absence of one ingredient someone decided to worry about, a muted colour palette on the box. None of that is dishonest, and none of it is verifiable. The gap is not specific to oral care either. It is the same word doing the same job on natural and non-toxic body washes, on shampoo, and on deodorant.

Cosmetic or Drug: How to Tell on the Shelf

Toothpaste is one of the few products that can be either, and US law splits them by intended use rather than by ingredient source. FDA defines cosmetics as articles applied to the body "for cleansing, beautifying, promoting attractiveness, or altering the appearance." It defines drugs, in part, as "articles intended for use in the diagnosis, cure, mitigation, treatment, or prevention of disease."

Cleaning teeth and freshening breath are cosmetic uses. Preventing cavities is a drug use. FDA names this exact case. It lists "toothpastes with claims to freshen breath and cleanse the teeth that contain fluoride" as a cosmetic and drug combination. Its example of an ingredient with a well-known therapeutic use is fluoride in toothpaste.

The consequence is printed on the package. 21 CFR 201.66 sets the labeling format for "all OTC drug products." It requires a panel titled Drug Facts, carrying Active ingredient, Purpose, Uses, Warnings, Directions and Inactive ingredients in that order. The anticaries monograph at 21 CFR Part 355 then fixes what may go in it. The only active ingredients it recognises are sodium fluoride, sodium monofluorophosphate and stannous fluoride. The indication it permits reads "Aids in the prevention of dental," followed by a choice of "cavities," "decay," or "caries."

So turn the box over. A Drug Facts panel with an anticavity purpose means the product sits inside that monograph. No panel means a cosmetic. It may clean very well, and it is not claiming to stop decay.

Where Fluoride Actually Sits

Worth stating plainly, because the natural aisle is often read as an argument against it. The anticaries case for fluoride is strong and old. A 2019 Cochrane review pooled 96 studies published between 1955 and 2014. It concluded that the review "supports the benefits of using fluoride toothpaste in preventing caries when compared to non-fluoride toothpaste." Its certainty rating for fluoride toothpaste reducing caries in children and adolescents was high to moderate, and moderate for adults of all ages.

Nothing in the natural aisle changes that finding. Going fluoride-free is a legitimate preference, and it is a tradeoff rather than an upgrade. What the "Natural" Ingredients Actually Do

Abrasives. Every toothpaste is abrasive by design. The monograph defines a dentifrice as "an abrasive-containing dosage form," and abrasives as solids added "to facilitate mechanical removal of dental plaque, debris, and stain from tooth surfaces." The ADA's Relative Dentin Abrasivity scale rates products against a reference abrasive set at 100, and anything at or below 250 is considered safe and effective. One caveat the marketing never carries: the ADA says the scale "should not be used to rank the safety of dentifrices." Values below 250, it adds, "do not correspond to potential clinical changes to enamel." A lower number is not the reassurance it reads as.

Charcoal. This is the one to be careful with. A literature review published in the ADA's own journal in 2017 found "insufficient clinical and laboratory data to substantiate the safety and efficacy claims of charcoal and charcoal-based dentifrices." Three of the studies it retrieved reported "deleterious outcomes (increased caries, enamel abrasion, nonquantified negative impact)." Its advice to clinicians was to tell patients "to be cautious." The ADA's whitening page lands in the same place. It cites a report in which charcoal and salt left teeth looking yellower, because brushing had removed enamel and exposed the dentin beneath.

Baking soda. It cleans, and that is roughly the whole claim. A 2019 systematic review from the dental faculty at Amsterdam found baking soda dentifrice beat control pastes on plaque in single-brushing studies. It then wrote that "the finding was partially substantiated in follow-up studies," and found no significant difference on gingival index scores. Plaque is not decay, and none of this is remineralisation. It is the same alkaline ingredient that makes baking soda deodorant good at odour and irritating for some skin.

Xylitol. Narrower than the packaging suggests. A 2015 Cochrane review found "some low quality evidence" that a fluoride toothpaste containing xylitol beat a fluoride-only toothpaste in children's permanent teeth. The rest of the evidence, it said, "is insufficient to determine whether any other xylitol-containing products can prevent caries." Note what was tested: xylitol added to fluoride, not xylitol instead of it.

Essential oils. These are the flavour in most natural pastes, and the same reason essential oils count as fragrance ingredients on a skincare label. The ADA lists them as the most common source of perioral contact dermatitis, stomatitis or cheilitis, naming spearmint, peppermint and cinnamon. It adds that they "may not be explicitly mentioned on the packaging, but may instead be among unspecified 'flavors.'"

Hydroxyapatite. The one fluoride-free active with randomised trials behind it, and the evidence is thin rather than absent. A 2025 systematic review in the Journal of Dentistry found no significant difference against fluoride on new or progressing lesions, drawn from four eligible trials out of 68 retrieved. Its authors sit at the University of Athens and the University of Bern, with no toothpaste manufacturer among the affiliations. That is worth checking yourself on this ingredient in particular.

Two routing lines are written into the monograph itself rather than inferred. Its directions for standard fluoride paste say: "Children under 2 years of age: Consult a dentist or doctor."
